1) A sixteen-year-old male presents to the hospital with complaints of nausea and vomiting. He states that the onset of the symptoms was last night. His vital signs are; blood pressure 132/85, pulse 101, temperature 101.1 and respirations 18. He is diaphoretic (sweating) and his face is erythematous(red). Upon assessment, you note that his skin turgor is poor. He is wearing sleep pants and a shirt, but reports feeling cold. As the nurse taking care of him you suspect a fluid volume deficit due to excessive output and no intake of fluids. You know that you can either attempt to get the client to drink some fluid or start intervenous fluid to provide the needed liquid. Based on your clinical judgment, you request an order for intervenors fluid from the provider. Additionally, you give him some medication to control the nausea and vomiting. After the IV has been running for a while, you reassess the client. Upon assessment, you notice that he appears more comfortable and his skin turgor has improved, his pulse is now 88, and his temperature is 99.2.
